There are three different services offered by Bendigo Health Case Management Services:

  • Acquired Brain Injury, Case Management
  • Assisted Community Living Funding
  • Flexible Funding

Acquired Brain Injury Case Management

Acquired Brain Injury (ABI) Case Management assists people with a moderate level acquired brain injury to remain living at home. A Case Manager provides a single point of contact for the person and their family to deal with the service system. Case managers work one on one with clients, families and carers to identify needed services and supports for the person.

 

Eligibility

 

The target group for this service are people aged 18 years and older who have:

  • a diagnosed ABI, and
  • who are non-compensable, and
  • who require moderate assistance to maintain independent living

Assisted Community Living Funding

 

Assisted Community Living funding may be used to access support services which can provide assistance with:

  • daily living activities such as household management or
  • participation in social or vocational activities.

Eligibility

 

The target group for this service are people aged 18 years and older who have:

  • a diagnosed ABI, &
  • who are non-compensable &
  • who require moderate assistance to maintain independent living.

Flexible Funding

 

Flexible Funding may be used to provide respite, top up services and fund social support groups.

 

Eligibility

 

The target group for this service is those who:

  • have a carer.
  • are non-compensable, and
  • who require moderate assistance to maintain independent living

To discuss a possible referral contact a Case Management Services Team Leader.

To make a referral send the Department of Human Services Service Coordination (Consumer Information, Consumer Consent and Summary and Referral Information) forms to a Team Leader. These forms are available on www.connectingcare.com or by calling our office.
