The hospital in the home program aims to provide individualised and holistic care to patients who require home based nursing care.
The program allows patients to receive ongoing treatment in the familiar surroundings of their own home. Each patient is individually assessed to determine his or her needs and a plan of care is coordinated in consultation with a team of health professionals. |
The aims of hospital in the home are to:
- Provide continuous patient care from the hospital to the home.
- Minimise the disruption to the patient's regular routine during their treatment and empower the patient and their family.
- Minimise family separation associated with hospital admission.
- Co-ordinate your care from hospital to home and monitor your progress.
- There is no cost to receive hospital treatment at home
Whilst you are in the hospital in the home program you are still considered an inpatient by Bendigo Health.
